To ensure we can appropriately accommodate students with special dietary needs and be in compliance with current USDA regulations, please submit a completed medical statement to your School Nurse using this Student Nutrition Meal Accommodations Request Form. A signature from both the parent/legal guardian and a medical authority is required on the form. Current USDA regulations no longer require annually updated medical statements. Dietary forms on file will remain active and valid until there is a request to modify or discontinue the request.

Meal pricing is not affected by dietary modifications. The cafeteria staff tries to be vigilant with children, but it is ultimately the parent’s responsibility to educate the child about what foods should be avoided.
