Application

**A student must have a full credit in Principles of Health Science as a prerequisite for Holland classes. Students with partial credit may be considered on an individual basis based on availability of summer school classes and skills. 

  1. A link to the online application will be sent to the email provided within 48 hours of the request. This link will come through an email from DocuSign.

  2. Students must attend a program meeting. See Letter of Understanding for more details. Please note all programs have fees that are subject to change. These fees and other requirements will be discussed at the program meetings.

Students will need the following documents to submit with their online application. 

  1. Social Security card

  2. Shot record- with TB results if possible

  3. Photo ID (driver’s license, military ID, or passport)

  4. Proof of group 1 background check (results are preferred, but receipt of payment will work – results are required before May 25th).

Give teacher recommendation form (obtain from principles teacher at home campus) to a core teacher for completion. This must be submitted to Ginger Held or Kristy Borton.
